Safeguarding Adults Reviews (SARs)
A Safeguarding Adults Review is carried out when an adult at risk dies or has experienced serious neglect or abuse, and there is concern that agencies could have worked more effectively to protect the adult.
A Safeguarding Adults Review is a multi-agency learning process. It aims to:
- identify and promote good practice
- encourage effective learning
- make recommendations for future practice so that deaths or serious harm can be prevented from happening again
SAR Referral
Any organisation can make a SAR referral to the Safeguarding Adult Board. The referral for consideration of a SAR should be made on the appropriate form which can be obtained by emailing BSAB@bradford.gov.uk.
Once completed, the form should be submitted to the Chair of the SAR Sub Group (via BSAB@bradford.gov.uk). All such referrals should be sent securely in line with the referrer’s organisational policy.
